Report of the Review of the Safety and Functionality of HMNZS Canterbury (2008)
Following two significant incidents involving the HMNZS Canterbury, including the tragic death of a crew member, an independent review was commissioned to review the ship’s operation, design and performance.
The Review did not identify any new design or performance issues beyond those already identified by Defence, and concluded that with some modifications HMNZS Canterbury will be fully functional. The review found that the ship was intrinsically safe and there was no connection between the ship’s design and performance and the crew member’s death.
